Do I qualify for Lilly Cares?
To receive free Lilly medications you must meet ALL program eligibility requirements. Some of the eligibility requirements for Lilly Cares are listed below. If you meet the criteria listed below, this does NOT guarantee eligibility for the program.

  • I am a permanent and legal resident of the United States and Puerto Rico.
  • My household income is under the Annual Income Limit.
  • I am NOT enrolled in or eligible for Medicaid or Veterans Administration Benefits.
  • If I am Medicare Part D eligible, I am enrolled in a Medicare Part D program.
  • If I am a Medicare Part D patient (except Forteo, Olumiant & Taltz patients), I have spent $1,100 on medications this calendar year.
  • My healthcare provider prescribed a Lilly medication available through Lilly Cares, and I am eligible for and have enrolled in Medicare Part D OR have no insurance.
  • My healthcare provider prescribed Humatrope and I am eligible for and have enrolled in Medicare Part D OR have no insurance OR my insurance does not cover the Lilly medication Humatrope. Patients with Medicaid or Veterans Administration benefits may apply. I have a “No Funding Letter” from Humatrope DirectConnect which states that I have no insurance benefits to pay for my Humatrope therapy. Contact Humatrope DirectConnect at 1-84Humatrope (1-844-862-8767) if you need this letter. A no funding letter is not required for Medicare Part D patients.

    New!
Applications for the Lilly Cares Foundation Patient Assistance Program may now be submitted online through a secure website.
Here's how the online application process works:

    1. A patient and their healthcare provider (HCP) each submit their portion of the application separately. Individuals type information directly in the online application form and then upload (attach electronic copies of) required documents.
    2. Once a patient or their HCP start the application process, Lilly Cares sends an email to the other party (patient or HCP) to request they complete their portion of the application and provide their electronic signature confirming consent.
    3. To submit a Lilly Cares application online, the person starting the application process must provide an accurate email address for both the patient and the HCP who will be completing the application. If the person starting the application process does not know the correct email addresses for both individuals, a paper version of the Lilly Cares application may be submitted and is available for printing from the Resources tab of this website or by calling Lilly Cares at 1-800-545-6962.

To avoid online application-related email communication from The Lilly Cares Foundation being sent to your junk email folder, please include DONOTREPLYLillyCares@LillyCareseservice.com in your approved email sender list settings.
